Case Manager (Supports Planner)

Job Overview Service Coordination, Inc. (SCI) is hiring Supports Planners to provide case‑management services and support to people with disabilities, medically complex needs, or other populations requiring assistance with Activities of Daily Living. The role involves building positive relationships, creating and monitoring Person‑Centered Plans of Service (POS), and coordinating resources to meet client goals. Responsibilities Advocate on behalf of clients, provide resource information, and educate them to become more self‑determined Provide education to clients and their support systems Teach coordination and advocacy for services Encourage empowerment and optimal independence Educate clients on the principles of self‑determination Collect information and documentation related to eligibility for Supports Planning services Meet with clients and/or authorized representatives to obtain information Gather supporting documentation Plan for and update Person‑Centered Plans of Service (POS) Plan for, facilitate, develop, and continually update Person‑Centered Plans of Service and POS tools that document service needs for eligible clients Engage in person‑centered planning with clients Develop and write POSs in collaboration with clients Conduct POS preparations with clients Research, identify, refer, and coordinate resources and services to achieve outcomes specified in the client’s POS and share resources with team members Provide information, make referrals, and assist clients Assist clients with transitions Complete required paperwork and follow up for waivers Identify services and collaborate with providers for institutional clients to transition into the community Monitor POS and conduct follow‑up activities Conduct on‑site visits to support and observe clients Conduct timely visits and contacts as defined in regulations Obtain progress information toward goals Identify new medical and health services and other personal needs Complete the Reportable Events (RE) as defined in regulations Apply or re‑apply for necessary programs or services Communicate with providers regarding resolution of concerns Assist with documentation to apply for medical assistance Adhere to SCI policies and procedures Meet performance utilization benchmarks to ensure compliance with SCI standards Maintain HIPAA records in secure locations Utilize tracking and time‑management tools Qualifications Education: Bachelor’s or Master’s degree in a human service field such as Human Services Administration, Psychology, Sociology, Social Work, Nursing, Counseling, or a related discipline. Experience: Experience working with people with disabilities preferred; experience with person‑centered planning preferred. Competencies Problem Solving – identify options and workable solutions. Stress Tolerance – maintain performance under pressure. Positive Work Attitude – approach work with a can‑do mindset. Physical Demands Standard office environment. Must remain stationary >95% of the time, operate a computer and related office equipment, occasionally lift 10–25 pounds, and be able to operate a motor vehicle. Vision and communication skills are required. Benefits and Compensation Base salary range of $64,395‑$79,861 for incoming Case Managers. Travel reimbursement and quarterly bonus potential.
