Case Manager (Supports Planner)

Service Coordination, Inc.

Position Summary Provide case management services and support to individuals with disabilities, medical needs, or other related conditions. Responsibilities include establishing positive relationships, developing Person‑Centered Plans of Service (POS), coordinating resources, and ensuring service quality and compliance with state mandates. Essential Duties Advocate for people served, provide education on self‑determination, and empower them to achieve independence. Collect and document eligibility information, obtain supporting documentation, and update POS. Develop, implement, and continually update POS and related tools in collaboration with clients. Research, refer, and coordinate resources to meet POS goals, assist with transitions, and complete required paperwork. Monitor POS progress through on‑site visits, timely contacts, and reporting of required events. Maintain compliance with SCI policies, HIPAA, and performance benchmarks; utilize tracking and time‑management tools. Required Competencies Problem Solving – identify workable solutions. Decision Making – evaluate alternatives. Job Knowledge – basic knowledge to perform the role. Achievement – pursue high goals. Minimum Qualifications Education: Bachelor’s or Master’s degree in a human service field (Human Services Administration, Psychology, Sociology, Social Work, Nursing, Counseling). Experience: Preferred experience working with people with disabilities and person‑centered planning. Physical Demands Standard office environment. Must remain stationary >95% of the time, operate a computer, and use office equipment. Ability to drive a motor vehicle and communicate effectively.
