Home Health Physical Therapist (PT)

We’re hiring a Home Health Physical Therapist (PT) who wants strong earning potential, autonomy in the field, and the opportunity to provide personalized care in a smaller, high-touch environment.

You’ll step into a home health role focused on evaluations, treatment, functional improvement, and helping patients regain mobility and independence in their own homes. You’ll manage your territory with considerable autonomy while joining an established organization with an existing patient base throughout Northern Connecticut.

• You are a licensed Physical Therapist in Connecticut or eligible for licensure
\n • You bring professional Physical Therapy experience
\n • Home health experience is strongly preferred
\n • You’re comfortable working independently in the field
\n • You enjoy helping patients improve mobility, function, and independence at home

Compensation & Benefits

• Base salary: $105,000–$130,000
\n • Additional productivity incentives available, with final structure being established
\n • Medical, dental & vision insurance available
\n • PTO: 15 days
\n • Paid holidays

\n • Mileage reimbursement: $0.725 per mile between patient homes
