Pine County seeks a qualified Case Manager to support clients with social, health and economic needs. You will assess, plan, coordinate services with a multidisciplinary team and connect clients to community resources. Telework may be considered for certain duties. Responsibilities include waiver coordination, documentation, and policy-compliant reporting.
