Supervisory Social Worker (Patient Aligned Care Team)

Summary The incumbent works for the Fredericksburg Health Care System (FxHCC) in Fredericksburg, Virginia, as a direct report to the Patient Aligned Care Team (PACT) and the Associated Programs Social Work Supervisor. Duties Provides supervision to the PACT social work team and manages the program needs of PACT social workers, collaborating with interdisciplinary leadership. Ensures PACT social work services and documentation meet national and local social work guidelines and adhere to Medical Center Policies and Standard Operating Procedures. Provides direct clinical social work services in the PACT department, including biopsychosocial assessments, case management, and crisis intervention. Performs other directly related duties as assigned. Schedule Work schedule: Monday‑Friday, 8:00 am‑4:30 pm. No telework or virtual work.
