If so, apply today!As a Clinical Team Manager, you'll contribute to our success in the following ways:Oversee patient care services, staff assignments, referrals, and care coordination to ensure patient needs are met.Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.Ensure timely start of care and ongoing compliance with applicable regulatory guidelines.Review and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness.Conduct regular OASIS reviews to support accurate documentation and quality patient care.Provide ongoing guidance, coaching, and support to direct care team members and assist with interviewing and hiring clinical staff.Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.What is Required?Associate degree in a Nursing related field, Bachelor preferred2 years of home care experience in Home Health or Hospice environment1 year of supervisory experience requiredCurrent, unrestricted RN license valid for the state of workAbility to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft OfficeAbility to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipmentMust possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.What's in it for you?Comprehensive insurance plans for medical, dental, and vision benefits including pet insurance401(K) with employer matchTuition reimbursement for full-time staff and continuing education opportunities for all employees at no costPaid time off, paid holidays, family, and pet bereavementFleet vehicle program (restrictions apply) or mileage reimbursement.* Benefit eligibility can vary by position and shift status.Equal Employment Opportunity: We are proud to be an equal opportunity workplace and comply with state and federal affirmative action requirements.
