About the Position
The San Luis Obispo County Sheriff's Office is seeking collaborative and adaptable professionals to join their team as a Pre-Release Care Manager (Behavioral Health Clinician III). This is an exciting and rewarding opportunity to work in forensic social work.
The California Advancing and Innovating Medi-Cal (CalAIM) Justice-Involved Initiative administered by the Department of Health Care Services (DHCS) mandates all state and local correctional facilities to provide targeted pre-release services to eligible persons prior to release from incarceration. This position will be working side by side with a dedicated team of Correctional and Healthcare staff seeking to improve the continuity of care for individuals being released into the community. The position will be located in the County Jail and will serve as a case manager for a group of individuals, responsible for creating reentry care plans and facilitating a warm handoff to a community-based care manager. The position will work in coordination with discharge planners and enhanced care management providers to ensure that pre-release services and care coordination occur as clinically indicated.
Although Behavioral Health Clinician is part of a class series of Behavioral Health Clinician I/II/III, the department is seeking to hire at the III level.
This recruitment is to fill a Limited Term, full-time position. Employees in limited term positions receive benefits equivalent to permanent full-time employees, therefore will be eligible to earn vacation, sick leave, health, and pension benefits. The current sunset date for this position is March 31, 2027. This position may be eligible for a $2.00 per hour differential for all hours worked in the jail facility.
