Volunteer AmeriCorps Patient Health Navigator

Patient Health Navigator – AmeriCorps Program Sea Mar Community Health Centers, a federally qualified health center (FQHC), seeks an AmeriCorps member to serve as a Patient Health Navigator. The role supports patients in accessing comprehensive health services, ensuring continuity of care, and promoting health education within the community. Key Responsibilities Schedule appointments and guide patients through the health system to overcome barriers. Initiate communication with patients and assist with referrals to address their holistic needs. Participate in mandatory trainings, service projects, and team‑building activities during the 10‑11 month full‑time term of service. Create informational materials and resource guides for patients, including posters, brochures, recipes, exercise information, and handouts. Prepare mailouts with appropriate materials and patient identifiers. Review electronic health records to identify patients requiring important health screenings and services; schedule appointments accordingly. Collaborate with clinic staff, quality improvement personnel, and other Sea Mar departments to identify patients needing screenings and to inform care teams about preventive care. Support rollout of additional preventive health screening and recall efforts. Participate in AmeriCorps site health project initiatives, such as community outreach, health interventions, and events on topics like smoking cessation, diabetes education, nutrition, healthy lifestyles, stress reduction, and chronic condition management. Increase public awareness of health‑coverage enrollment opportunities by planning and conducting outreach activities within the community and at Sea Mar‑sponsored events. Assist in the development and distribution of informational/promotional materials and maintain referral relationships with local organizations, agencies, institutions, and businesses. Provide translation/interpretation services as needed. Qualifications Strong or native proficiency in Spanish (bilingual Spanish strongly preferred). Reliable transportation. Ability to work independently and as part of a multidisciplinary team. Excellent communication skills with diverse populations. Commitment to a 10‑11 month AmeriCorps service term. Compensation and Benefits AmeriCorps members receive a prorated stipend of up to $20,400 for the 1,700‑hour service year, paid bi‑weekly. A reduced living allowance applies for members completing fewer hours. Upon successful completion, members receive a maximum education award of $7,395.
