Registered Nurse (RN) Supervisor - Women's Health & Pediatrics

We are seeking an experienced and compassionate Registered Nurse (RN) Supervisor to join our Certified Home Health Agency (CHHA) team, supporting our Women's Health and Pediatrics Program . In this leadership role, you will oversee and support field nurses while ensuring the delivery of high-quality, patient-centered care to women and pediatric patients in the home setting.

Key Responsibilities

Clinical Oversight & Patient Care

• Provide clinical supervision and guidance to field nurses caring for Women's Health and Pediatric patients.
• Conduct patient assessments, including admissions and supervisory visits.
• Ensure individualized plans of care are appropriate, updated, and followed.
• Monitor patient progress and intervene as needed to ensure optimal outcomes.
• Collaborate with physicians and interdisciplinary team members to coordinate quality care.

Staff Supervision & Support

• Supervise, mentor, and support RN field staff.
• Conduct performance evaluations and provide ongoing coaching and feedback.
• Assist with orientation and training of new nursing staff.
• Serve as a clinical resource for Women's Health and Pediatric cases.
• Foster a positive, collaborative, and patient-focused work environment.

Care Coordination

• Coordinate care with physicians, therapists, home health aides, and other interdisciplinary team members.
• Participate in case conferences and care planning meetings.
• Ensure services are delivered in accordance with physician orders and agency standards.

Documentation & Compliance

• Review and ensure the accuracy of OASIS documentation and clinical records.
• Maintain compliance with all federal, state, and agency regulations.
• Oversee timely completion of required documentation and quality assurance activities.

On-Call Responsibilities

• Participate in rotating on-call coverage.
• Provide clinical guidance and support to field staff during on-call hours.

Qualifications

• Active Registered Nurse (RN) license in New York State.
• Experience in Women's Health, Pediatrics, Home Health, or a related clinical specialty required.
• Minimum of 2 years of nursing experience required.
• Previous supervisory or leadership experience preferred, but experienced home care field nurses looking to advance into leadership are encouraged to apply.
• CHHA experience is a plus.
• Knowledge of OASIS and home health regulations preferred.
• Excellent clinical judgment, communication, organizational, and leadership skills.
• Valid driver's license and ability to travel throughout Brooklyn and Queens .

Physical Requirements

• Ability to travel to patient homes, including walk-ups and varied home environments.
• Ability to stand, walk, bend, and lift as needed throughout the workday.
• Ability to lift up to 50 pounds independently and more with assistance when necessary.

Health Requirements

• Completion of pre-employment and annual health assessments.
• Compliance with all New York State immunization and health requirements.
