Position Details Specialty: Physical Therapist Start Date: 06/01/2026 Duration: 20 weeks Hours per week: 40 (8‑hour days) Employment Type: Travel Schedule Day shift: 5 days × 8‑hour day (08:00‑17:00) Every 3rd weekend required Responsibilities Assess and document patient health status, focusing on cardiopulmonary, geriatric, neuro, and orthopedic needs. Provide patient and family education on medications, home exercise programs, and safety/injury prevention. Perform skilled nursing visits at patient homes, with a minimum of six visits per day. Utilize equipment and devices as necessary, including crutches, gait belts, and supplemental oxygen delivery. Identify signs and symptoms of physical, sexual, and psychological abuse in patients. Required Experience / Certifications / Licensure Valid state nursing license or a compact license. BLS certification required. At least two years of home‑health experience mandatory; OASIS training preferred. Valid driver’s license and access to a car for home visits. COVID vaccination required or exemption form on file.
