Responsibilities:

• Provide personalized care and support to clients in their homes, including assistance with daily living activities, medication management, and companionship

• Develop and maintain strong relationships with clients, families, and caregivers to ensure exceptional care and communication

• Collaborate with clients, families, and healthcare professionals to create personalized care plans

• Perform light housekeeping, meal preparation, and other household tasks as needed

• Maintain accurate and timely records of client care and services

• Participate in ongoing training and professional development to stay up-to-date on best practices and industry developments
