The Community Liaison serves as a dedicated resource to facilities, patients, families, and referral partners by ensuring a seamless transition into hospice care and an exceptional customer experience from admission forward. This role proactively rounds in facilities on the day of or following admission to verify that all elements of the patient start-of-care process are complete, including required documentation, facility compliance requirements, medications, durable medical equipment (DME), supplies, and admission materials.

Acting as a bridge between hospice operations, clinical teams, and community partners, the Community Liaison identifies and resolves potential service gaps before they impact patient care or customer satisfaction.

An Average Day: (Includes, but not limited to)

• Build and maintain strong relationships with healthcare facilities, referral partners, patients, and families to ensure an exceptional hospice experience.
• Conduct facility rounds following patient admissions to verify admission materials, medications, equipment, and supplies are in place and that all care coordination needs have been met.
• Serve as a liaison between facilities, clinical teams, and hospice leadership to proactively identify and resolve service concerns.
• Provide education and support to facility staff, patients, and families regarding hospice services and philosophy of care.
• Promote Bristol Hospice's commitment to quality, responsiveness, and exceptional customer service through consistent facility presence and partnership.
• Other duties as assigned

Requirements:

• One to two years of experience in Hospice Sales preferred
• Must be flexible in work hours
• Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
