Occupational Therapist - Travel Contract

Make a meaningful difference as a travel OT. We are seeking a dedicated and compassionate Occupational Therapist to help patients with injuries, illnesses, or disabilities regain or improve daily skills while working at a skilled nursing facility in San Jose, CA.

Minimum Requirements

Bachelor's, Master's, or Doctoral Degree in Occupational Therapy from an accredited program.
Active California Occupational Therapy License required to start the assignment.
BLS certification from American Heart Association or American Red Cross may be required. (Preferred: current BLS.)
Experience working in skilled nursing facility or long-term care settings preferred.
Experience providing treatment to geriatric and medically complex patients preferred.
Documentation experience with electronic medical records required.
Ability to perform standard OT evaluations, create individualized treatment plans, and carry out restorative and ADL training.

Facility Type: Skilled Nursing Facility.
Shift Name: Days.
Shift Hours: AM 8 (5x8 schedule).
Assignment Length: 13 week assignment.
