Summary

This position is established for the purpose of providing adult protective services to vulnerable adults.

Essential Functions

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  1. Performs intake screening and processes reports, including after- hours calls alleging abuse, neglect, and/or exploitation.
  2. Inputs intake screening data and other data as necessary into appropriate documentation and data collection software, as needed.
  3. Investigates, as directed, reports of abuse, neglect, and/or exploitation; conducts home visits to observe surroundings and interview reported adults, caregivers, and alleged perpetrators.
  4. Assesses living arrangements, physical capabilities, cognitive abilities, medical requirements and financial resources.
  5. Develops protective service plans and provides on-going follow-up and monitoring to reduce risk and increase safety of reported adults.
  6. Recommends, prepares and attends legal interventions, such as guardianship, power of attorney, protective services, and /or placement, as necessary to ensure individuals safety and well-being.
  7. Provides information and referrals to local agencies and organizations; coordinates community resources.
  8. Interacts with health care providers, community organizations, law enforcement, and government agencies.
  9. Completes case documentation using appropriate computer applications to document events, record interviews and other relevant information.
  10. Corresponds verbally and in writing with associated parties to ensure all those affected are informed.
  11. Responds to reported elder-related emergency situations outside normal agency operating hours.
  12. Provides customer service by responding to general adult protection inquiries, as well as recording specific incidents and offering guidance.
  13. Prepares case documentation for administrative or judicial procedures as required and testifies in court as necessary.
  14. Prepares for and facilitates quarterly Interdisciplinary Team (I-Team) meetings. Prepares and present information related to services provided by APS.
  15. Meets procedural deadlines governing management of APS.
  16. Prepares case reviews, correspondence and complies statistical information for state and county reports.
  17. Attends workshops and training sessions to stay current on relevant legislation. Maintains current manuals, desk aids and instructional guides.

  20. Represents the Area Agency at community and special events as requested.

Requirements:
  1. Associate degree in human services or related field OR commensurate experience OR Bachelor's degree from an accredited college or university in Social Work, Psychology, Sociology, Gerontology, Nursing, Health Science, Health Care Administration, Human Resources, Political Science, Anthropology, Human Services, Public Administration, Education, Counseling, Criminal Justice, or a closely related field.
  2. Ability to travel in a multi-county service area and access private homes and facilities, which may not be readily accessible
  3. Professional telephone and administrative skills.
  4. Presentation skills
  5. Knowledgeable in Microsoft Word and Excel.
  6. Experience with office procedures and related equipment.
  7. Strong problem-solving skills
  8. Strong written and oral communication skills.
  9. Ability to make sound decisions and demonstrate critical thinking skills.
  10. Ability to communicate professionally, respectfully and effectively.
  11. Ability to prioritize, organize and maintain accuracy in a flexible job environment.
