Duties Description Will function as the HS1 supervisor at the site. Will provide supervision and monitoring of direct care staff assigned to the site.
PRIMARY DUTIES AND RESPONSIBILITIES:The primary duties for this position include, but are not limited to the following:
The Habilitation Specialist 1 (HS1) supervisor will assist the Habilitation Specialist 2 (HS2) with tasks assigned on a daily/ongoing basis, including but not limited to Day Habilitation documentation and delivery. Supervision and knowledge of scheduling is an integral part of this position. This includes supervision of staff assigned to the site, ensuring ongoing evaluation of staff performance, and training of staff.
The HS1 will be responsible to work with the HS2 in monitoring compliance with all applicable regulations and Medicaid billing requirements. Ensuring the delivery of program services, supervision and support of individuals in day habilitation.
The HS1 supervisory position requires computer skills to complete job assignments which include, but is not limited to, accessing and using EHR, TIMES, SLMS and SFS computer programs.
Completion of other assigned tasks based on the operational needs of the site location.
Consideration will be given to the candidate who demonstrated skills in curriculum development and implementation, experience/success in establishing linkages with the community to promote individuals' inclusion and expertise relevant to the population to be served.
Day Habilitation programs offer the most integrated programs and plans in the community setting. Therefore, the majority of the day habilitation will be expected to not occur 'within the walls' of the program building. All programs are based on individual's needs and Person-Centered Planning. Person identified will be expected to have knowledge of community available opportunities for participation. Person identified should have knowledge of Person-Centered Planning and how we will work towards enhancing community integration in Day Habs. Additional duties include but not limited to:
• Develop and update Individuals Safeguard Summary Plans (SSP).
• Develops and updates residential Staff Action Plans (SAP) with the individual, family/advocate, Program Planning Team, Care Coordinator and service providers.
• Provide weekly/monthly review of service notes documentation for Medicaid Compliance Billing
• Participate in the development and review of Support and Risk Accountability Plans (SRA). Present at SRA meetings.
• Train residential staff on the implementation of Staff Action Plan outcomes and documentation of individual's response to services.
• Monitor the implementation of Safeguard Summary Plan (SSP), Staff Action Plan (SAP) and Support and Risk Accountability Plan (SRA).
• Review all documentation, assess status and progress of the Staff Action Plan (SAP), Support and Risk Accountability Plan (SRA) and Safeguard Summary Plan (SSP). Revise as needed to assure that plans continue to reflect the individual's abilities and need for protective oversight and supports. Review billing documentation monthly.
• Submit and Present information for the Behavioral Intervention Rights Committee (BIRC).
• Submit Clinic Referrals; Disseminate copies of completed clinic reports.
• Participate in the Life Plan review meetings, House and Team Meetings, Special Meetings and take minutes.
• Participate in Programmatic Self-Assessment and Regulatory Plans of Corrective Action (POCA) development, implementation and monitoring as relevant to the QIDP role.
• Participate with other team members in a variety of individual, team and continuous improvement meetings and forums as scheduled to promote comprehensive and individualized services.
• Must have the ability to adjust work hours to meet operational/supervisory needs including but not limited to Life Plan Meetings, trainings, and off shift visits to the houses on the team.

Minimum Qualifications Competitive Minimum Qualifications:
Must be on the Civil Service eligible list for this location.
Transfer Qualifications:
Must be in title or eligible for transfer under Section 70.1 in a designated title determined by the New York State Department of Civil Service.

A Bachelor's or higher-level degree in a human services field*
*For the purpose of these qualifications, a degree in a human service field includes the professional disciplines of audiology, community mental health, nursing, nutrition, occupational therapy, physical therapy, psychology, rehabilitation counseling, vocational counseling, social work, speech/language pathology and therapeutic recreation (including concentrations in art, creative art, dance, drama, music, or physical education.) Also included are related academic disciplines associated with the study of human behavior (e.g., gerontology, human services, sociology, speech communication); or human skills development (e.g., counseling/guidance, education, human development.) Examples of non-qualifying degrees include but are not limited to: criminal justice, political science, liberal arts, and business. Additional Comments PREFERRED QUALIFICATIONS: Qualified Intellectual Disabilities Professional (QIDP) Status.
Preferred qualifications are at least one year's experience providing services to persons with developmental disabilities AND either; a master's degree in a field of psychology, a bachelor's degree in a human services discipline, a psychology license currently registered with the NYS Education Department, a nursing (or licensed practical nursing) license currently registered with the NYS Education Department, or a master's degree in social work with current certification and registration with the NYS Education Department as a social worker.
