Remote Transitions of Care RN - Virtual Case Manager

Elevance Health

A leading health company is seeking a Transitions of Care RN to participate in patient education and health coaching. This virtual role offers full-time flexibility while requiring in-person training. Ideal candidates will have an AS in nursing and a minimum of 2 years of relevant experience. Responsibilities include conducting assessments and coordinating coaching plans. A compact RN license and bilingual skills are preferred.
