The Clinic Bridge Navigator works to coordinate patient care by providing a “bridge” between the medical providers, their clinical team, and clerical staff per established clinic protocols.  This team member works with all clinical staff in the delivery of patient care.  This is a full time, 40 hour per week position located at the Medical Clinic.

QUALIFICATIONS

Minimum:

•             High school diploma or equivalent required.  

•             Computer skills including Outlook, Word, and Excel required. 

•             Current CPR certification or obtained within 2 months of hire.

Preferred:

•             Registered (RN) or Licensed Practical (LPN) Nurse

•             Two years of clinical experience preferred.

•             Background in medical and pharmacology terminology preferred.

•             Excellent organizational, customer service and communication (written and verbal) skills necessary.

Roles and Responsibilities:

•             Manages patient prescription refills from a variety of sources, fax system and messages filtered through reception.  Disseminates refill requests and obtains insurance prior authorizations for prescriptions when indicated.

•             Collects pertinent records regarding ordered referrals and submits information to the appropriate facility.  Obtains prior authorization from insurance companies for ancillary services when indicated.  Places external referral order in electronic health record (EHR) for referral tracking purposes.  Maintains clear & frequent communication with patients regarding referral status.  

•             Assists with scheduling ordered consults and procedures that initiate from the clinic.  Coordinates with patient financial services (PFS) to ensure patients financially qualify for services.  Maintains clear & frequent communication with patients regarding procedure scheduling status.

•             Tracks all referrals and assures follow-up records are uploaded into the patient’s chart. Patient referral declinations are documented in the chart, and the referral will be “canceled/DC” in the EHR.  

•             When indicated, obtains insurance prior authorization for patient imaging studies, including CTs, MRIs, and echocardiograms.  Once authorization has been obtained, patient is scheduled either by Melham Radiology Department or by external facility.

•             Participates in mandatory training for EHR and demonstrate basic competency in the functions applicable to their role.

•             Works directly with providers and other clinic/hospital staff

•             Cooperates with other personnel to achieve department objectives.

•             Performs other duties as assigned.
