Our facilities function as Skilled Nursing Facilities (SNFs) that offer:

• Long-term care for residents needing continuous skilled support and compassionate daily assistance.
• Short-term, sub-acute rehabilitation for individuals recovering from surgery, illness, or injury.
• Collaborative care through a team-oriented approach involving nursing, therapy, and medical staff working in unison.
• A diverse patient demographic that includes those with orthopedic, neurological, cardiopulmonary, and post-acute needs.

The patient mix may vary slightly by location. As a therapy professional, you will assist patients in achieving functional improvements and enhancing their quality of life within a supportive, patient-focused setting.

The Speech Language Pathologist (SLP) delivers direct rehabilitation services in speech therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.

• Must be a graduate of an accredited speech therapy program (minimum MS or MA SLP).
• ASHA certified (CCCs) OR eligible for a 9-month Clinical Fellowship Year (CFY) to obtain ASHA certification (CCCs) and/or meet ASHA credentialing criteria. Once obtained, ASHA credentialing must be maintained.
• Currently registered/licensed in the applicable state, with an active license in good standing throughout employment.
• One (1) year of experience is preferred; new graduates are encouraged to apply .
• CPR certification is required upon hire or must be obtained during orientation. CPR certification must remain current throughout employment.

• Engage in community health initiatives/projects as appropriate.
• Proficient in Microsoft Word, Excel, and email communication.
• Collaborate with patients, families, and support departments to effectively plan for patient needs.
• Exhibit proper body mechanics at all times.
• Demonstrate the ability to make independent decisions when necessary.
• Familiar with practices, procedures, and regulations governing operations in a post-acute care facility.
• Implement and interpret departmental programs, goals, objectives, policies, and procedures.
• Perform competently in all required areas, including patient rights, safety, and sanitation.
• Maintain professional relationships with colleagues, vendors, and others.
• Ensure confidentiality of all proprietary and confidential information.
• Understand and adhere to organizational policies, including those related to harassment and compliance.
• Exhibit integrity and professionalism by following the organization's Code of Conduct and completing mandatory compliance training.

• Establish, assess, and adjust realistic, measurable, timely, and functional goals.
• (SLP CCC only) Oversee and evaluate care provided by SLP CFYs.
• (SLP CFY only) Follow speech therapy treatment plans under the supervision of the SLP CCC, in accordance with Federal and State guidelines.
• Document appropriately and in a timely manner.
• Utilize therapy software accurately and effectively.
• Provide excellent customer service and maintain a positive attitude towards patients.
• Assist in the safe evacuation of patients.
• Demonstrate reliable and consistent attendance.
• Employ reasoning skills and sound judgment.
• Communicate effectively and collaborate productively within an interdisciplinary team.
• Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during work hours.
• Read, write, speak, and comprehend the English language.
