Care Transition Nurse: Acute-to-Home Coordination

Position Summary The Transition of Care Nurse provides consultative and coordinating functions in the transitioning of patients from the acute care facility to home or alternative care sites. This position works in conjunction with the Nursing and Medical/Case Management departments of the hospital and at the direction of the management of AtlantiCare LIFE Connection. The Transition of Care Nurse also is responsible for the coordination of care of all AtlantiCare LIFE Connection participants currently residing in an acute care setting. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation. Qualifications Education: Graduate of an accredited school of Nursing required; Bachelor’s Degree in Nursing preferred. License: Current Registered Nurse licensure in New Jersey, valid New Jersey Driver's license, proof of registration and insurance, BLS certification required. Experience: Minimum 1 year working with frail or elderly, 1 year as Home Health or Long Term Care Nurse or equivalent, 2 years medical/surgical experience. Proficiency in Clinical Applications preferred; training provided within 30‑60 days. Performance Expectations Demonstrates the competencies as established on the Assessment and Evaluation Tool for this position. Work Environment Potential hazards: exposure to blood borne pathogens, infectious diseases, hazardous substances, and potential injury. Physical demands include reaching, stooping, kneeling, crouching; extensive standing, sitting, walking; lifting 25‑50 lbs frequently, 50‑100 lbs with assistance occasionally; traveling to company off‑site locations expected; heavy mental stress demands; handling large numbers of interactions with multiple individuals and constant interruptions.
