Healthcare Social Worker — Patient Advocate & Care Planner

Life Care Centers of America

Life Care Centers of America is seeking a Social Worker to plan, organize, and implement social services for patients in a post-acute care setting. You will advocate for patients, educate staff about patient rights, and ensure timely charting and documentation. Requirements include an active state license, a bachelor’s degree in a human services field, and 2 years of health-care social work experience preferred.
