Job Type: Crisis Rate | Contract Only
Hours: 40 hours/week
Shift: 8-Hour Day Shift | MonFri (8:00 AM 4:30 PM)
Contract Dates: Feb 16 May 16, 2026
Position Overview
Facility is seeking an experienced RN Hospice Case Manager to support the Sheboygan County hospice team, with occasional coverage in Manitowoc County. This is a home hospice case management role managing a steady caseload of adult end-of-life patients across homes, ALFs, and SNFs.
Caseload & Patient Population
- Caseload: 1315 patients
- Setting: Home, Assisted Living Facilities, Skilled Nursing Facilities
- Patient Type: Adults only End-of-Life / Hospice
- Ratio: 1:1
Required Experience
- 2.5+ years of hospice-based case management (REQUIRED)
- Prior home hospice experience
- Previous admission experience required
- EPIC Remote Client experience REQUIRED
- Travel experience preferred
- First-time travelers accepted if hospice + EPIC Remote experience
Required Licensure & Certifications
- Active RN License (Compact preferred)
- BLS
- Valid Driver's License & Auto Insurance

Schedule & Expectations
- Weekend Rotation: One weekend per month
- On-Call: One call shift per month
- Holidays: None
- Floating: Anywhere within Sheboygan & Manitowoc counties
- RTO: Manager approval required

- Up to 1 week, based on clinician comfort level
