Wellness Coach - Supportive Housing
Tandym Group
A New York City-based nonprofit services organization is seeking a new Wellness Coach for a supportive housing role in the Bronx. In this role, the Wellness Coach will be responsible for supporting clients in accessing essential community services and managing wellness processes.
About the Opportunity:- Schedule: Tuesday to Saturday or Sunday to Thursday
- Hours: 9am to 5pm
- Setting: Supportive housing site
- Developing service plans
- Writing progress notes
- Providing case management services
- Organizing and leading groups and events
- Connecting clients with community resources
- Performing other duties, as needed
- Bachelor's Degree
- Working knowledge of AWARDS and other computer programs
- Strong skills in Incident Reporting
- Great interpersonal and communication skills
- Self-initiative and accountability
- Teamwork and collaboration skills
- 3+ years of experience in Behavioral Health
- Bilingual (English and Spanish)
- Experience with Mental Illness and Homelessness
- Competency in Peer Counseling
- Experience in a Supervisory role
