Responsibilities include: Performing and documenting comprehensive patient assessments according to scope of practice and hospital policies. Considering the patient’s culture, age, and developmental level in assessments. Identifying patient risk factors and presenting complaints. Collecting objective and subjective data. Verifying initial admission assessments. Developing problem lists and prioritizing patient needs based on inter-professional input. Designing actions and interventions relevant to the patient’s context. Determining desired outcomes in coordination with patients and families. Implementing patient care therapies and procedures within established policies and guidelines. Explaining procedures and treatments to patients and families. Initiating actions to alleviate patient stress and discomfort. Recognizing and communicating changes in patient conditions. Documenting ongoing assessments and responses to treatments. Delegating tasks appropriately and overseeing team members. Monitoring accuracy and completeness of delegated patient care. Identifying and addressing educational needs of patients. Facilitating timely patient discharge processes. Providing input into patient/nurse assignments based on staffing and patient acuity. Actively participating in unit-based activities and promoting organizational values. Creating a positive and healing environment through supportive interactions.
