As a Day Service Specialist, you’ll develop and implement community-based, person-centered day service plans . You’ll build strong relationships with the individuals you support, while also engaging with their teams and the broader community. What You’ll Do: Plan and coordinate individualized community activities to promote independence and social interaction. Provide direct support to clients, including personal care, medication administration, and use of adaptive equipment. Document daily progress and services provided. Transport clients using a company-provided vehicle. Engage in community outreach and utilize technology to support your work.
