Role Description The Clinical Liaison is a full-time, hybrid role based in the South Bend–Mishawaka Region, with some work-from-home flexibility. This role is responsible for coordinating referrals, assessing patient needs, and facilitating smooth transitions into Healthwin’s rehabilitation and nursing services. The Clinical Liaison collaborates with hospitals, physicians, home care agencies, and other referral sources to communicate clinical information, eligibility, and service options. Daily tasks include conducting clinical evaluations, explaining programs and services to patients and families, and maintaining accurate documentation. The role also involves developing relationships with community partners, supporting business development efforts, and representing Healthwin in outreach activities. Qualifications Case Management and Home Care skills to assess patient needs, coordinate services, and guide individuals and families through care transitions. Rehabilitation experience to understand post-acute care, therapy services, and clinical pathways that support recovery and quality outcomes. Sales and Business Development skills to build and maintain referral relationships, promote Healthwin’s services, and support census growth. Strong communication, interpersonal, and collaboration skills to work effectively with healthcare teams, patients, families, and community partners. License or certification in nursing, social work, therapy, or related clinical field preferred; prior experience in post-acute, skilled nursing, or rehabilitation settings is beneficial. Ability to work independently, manage multiple priorities, and use electronic health records and common office software. Valid driver’s license and reliable transportation for local travel within the South Bend–Mishawaka Region. #J-18808-Ljbffr
