Care Coordinator/Social Worker

Social Worker Act as a liaison and assist in coordinating resources for the care of health plan members with multiple co‑morbidities and/or psychosocial needs. Provide counseling and assistance to patients and their families with personal, financial, ethical, and environmental difficulties that predispose them to illness. Advocate for members and their families by linking them to community resources and financial assistance programs. Collaborate with an interdisciplinary care team that includes physicians, nurses, case managers, pharmacists, laboratory technologists, other social workers, dietitians, and educators. Coordinate patient services through an interdisciplinary process, providing clinical and psychosocial care through the continuum of care. Initiate discharge planning at admission and reassess throughout hospitalization. Offer direct social work case‑management services to patients, families, and hospital staff to ensure appropriate assessment, intervention, support, and referral. Coordinate services with other members of the healthcare team and community-wide resources. Respond to information and referral needs of the community. Provide crisis intervention, mental‑health diagnosis understanding, brief counseling, collaborative vendor relationships, resource identification, and facilitation of meetings with the care team. Problem‑solve and promote patient wellness. Schedule: Monday – Friday, 8:00 AM – 4:30 PM / 12:00 PM – 8:30 PM Primary Responsibilities Coordinate patient care processes to achieve desired quality outcomes and control appropriate resource utilization. Provide skilled interventions for patient/family support and crisis resolution; facilitate problem-solving, decision-making, advocacy, and mediation of risk factors and psychosocial issues. Assess need for post‑hospital services and community resources at discharge. Develop care and management plans for frequent emergency department patients in partnership with physicians, ED providers, and community resources. Support patients, caregivers, and families with psychosocial or behavioral needs. Educate patients/families on available options and care choices. Report child and adult abuse and neglect to community agencies as mandated by law. Assess patients who may have experienced violence and make appropriate reports and referrals. Collaborate with the Care Team on care planning and discharge resources. Involve staff at the next level of care when appropriate to promote continuity of care. Interface with Department Leaders, Risk Management, and patient representatives to identify potential QA or risk issues. Participate in departmental quality improvement projects. Initiate all necessary paperwork for reports, plans of care, referrals, and community resources. Coordinate and discharge plan for all age groups, including unborn children through geriatric populations. Familiarize with and comply with all regulatory requirements, including CMS and Joint Commission Standards. Required Qualifications Bachelor’s degree in social work (BSW). Social Work Certification. At least 2 years of experience working in a healthcare setting. At least 2 years of experience/knowledge in healthcare terminology. Ability to effectively read, write, speak, process cognitively, and provide emotional support while performing duties assigned. Ability to work onsite at hospital location(s) in Waukesha, WI regional area. Preferred Qualifications Master’s degree in social work. Clinical social work experience. Hospital discharge planning experience. Soft Skills Critical thinking skills. Excellent written and oral communication skills. Strong oral and written communication skills. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to salary, we offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (subject to eligibility requirements). The hourly pay for this role ranges from $49,700 to $88,800 per hour based on full‑time employment. We comply with all minimum wage laws as applicable.
