Care Coordinator - Children's Long‑Term Support (CLTS) Program Waukesha County, WI Full‑Time | M‑F, First Shift | Remote Flexibility 23.70$/hr Lutheran Social Services of Wisconsin & Upper Michigan is seeking a compassionate and organized Care Coordinator for our CLTS Waiver Program team in Waukesha County ! As a Care Coordinator, you will be the primary point of contact for families, helping them access and navigate essential services and ensuring their children receive the support they need. What You’ll Do Assess children’s functional abilities using approved tools. Develop and implement individualized service plans with families and providers. Facilitate team meetings and coordinate services based on family‑centered goals. Maintain accurate documentation and meet all regulatory timelines. Collaborate with internal teams and external agencies to support families. Testify in legal proceedings when required. Communicate clearly with families, providers, and team members. Participate in staff development, training, and supervision. Minimum 1 year of experience working with children with disabilities. Bilingual fluency in Spanish is preferred but not required. Proficient in computer systems and electronic health records. Strong interpersonal and organizational skills. Valid driver’s license and reliable transportation (MVR check required). Work Environment Community‑based with daily travel required.
