As a Case Manager you will work collaboratively to engage, educate, communicate, and coordinate care with clients, behavioral health services, medical providers, community resources, and others in ensuring that all services prescribed in the individualized treatment plan are implemented. You will perform all case management functions associated with a caseload including outreach and follow-up, client transportation, completing assessment and service planning processes, and identifying the need for further or specialty evaluations.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Conduct intakes, interviews, and screenings of prospective clients.
• Conduct assessment of clients’ addiction behaviors needs for socialization and other life changes.
• Working closely with clients to develop treatment plans and make referrals as needed.
• Maintain complete chart notes; maintain confidentiality and adhere to all HIPAA policies.
• Facilitate and lead groups.
• Attend and participate in all required agency meetings and in-service training
• Other duties as assigned.
• Understand and implement individual treatment plans, record patient behavior, provide a safe supportive environment for the patients.

COVID-19 considerations:
All clients and staff are required to wear a face mask in group settings

Education:

• Bachelor degree preferred in a related field

License/Certification:

• CPR Certification, ID, Fingerprint & clearance card (Preferred)
