We are currently seeking a Clinic Director to serve as the on-site administrative leader of our clinic in Perris, CA. This position will report to the OTS Regional Director of Clinic Operations and will work in collaboration with clinic leadership and OTS leadership.
Indian Preference applies. Qualified Indian (American Indian or Alaskan Native) applicants will receive preference in accordance with 25 U.S.C. § 5323 and 25 U.S.C. § 472a.
We are looking for a person who has supervisory and leadership experience and training in treating substance use disorders, primary care, and behavioral health. Experience with multicultural populations is highly desired. The ideal candidate will be patient, humble, and determined, along with demonstrating agility, optimism, organization, and empathy in their work The Clinic Director is responsible for day-to-day operations of the clinic and is a key leader in the overall management of the program. This individual will develop, manage, report on, and be accountable for the daily operations of the clinic and its staff in their entirety. The Clinic Director will be a main contact with community leaders addressing substance use needs within and around Perris. In this role, the Clinic Director will in tandem with the Clinic Medical Director in the areas of human resources, finances, and general accountability.
THE DYAD MODEL The Clinic is led through a dyad leadership model consisting of the Clinic Director and the Medical Director. This partnership combines clinical expertise and operational leadership to ensure the delivery of high-quality, patient-centered care while maintaining regulatory compliance, financial stewardship, and organizational effectiveness. The dyad model is built on shared accountability, mutual respect, transparent communication, and collaborative decision-making. Together, the Clinic Director and Medical Director are responsible for advancing the clinic's mission, strategic objectives, patient outcomes, operational performance, and regulatory compliance. The dyad model is supported by OTS leadership, including those mentioned above.
The Clinic Director and Medical Director jointly share responsibility for:
- Establishing clinic goals, priorities, and strategic initiatives.
- Maintaining compliance with federal, state, accreditation, and organizational requirements.
- Ensuring safe, effective, and whole-person care.
- Optimizing patient access, engagement, retention, and treatment outcomes.
- Workforce recruitment, retention, development, and staff engagement.
- Quality improvement and performance management initiatives.
- Program development and service expansion.
- Risk management and problem resolution.
- Fostering a culture of accountability, collaboration, respect, and continuous improvement.
Operations & Service Delivery
- In partnership with site Medical Director, direct all clinic administrative operations including dosing, counseling, medical services, care coordination, and integrated primary care and wraparound services (behavioral health, case management, peer support, transportation, housing and benefits navigation)
- Own daily census, dispensing window efficiency, appointment throughput, wait times, and no-show/retention performance against established targets
- Ensure appropriate staffing coverage, provider scheduling, and coverage plans across all clinical and support roles
- Manage facility operations, safety, security, and vendor relationships
- Maintain continuous compliance with 42 CFR Part 8, 42 CFR Part 2, SAMHSA OTP certification requirements, DEA registration and controlled substance handling, and all applicable state opioid treatment authority (SOTA) requirements
- Lead accreditation readiness, survey preparation, and corrective action plans for applicable accrediting bodies, which may change from time to time; maintain mock survey and continuous-readiness discipline rather than cyclical scrambles
- Ensure compliance with HIPAA, OSHA, CLIA (where applicable), FTCA/HRSA requirements where relevant to primary care operations, and tribal health program requirements under Title I/Title V self-determination agreements
- Oversee medication inventory, diversion control, take-home determinations, and controlled substance reconciliation in partnership with the medical director and pharmacy
- Serve as site lead for internal audits, external inspections, and licensure renewal
- Partner with the site Medical Director and clinical leadership to ensure evidence-based MOUD delivery (methadone, buprenorphine, naltrexone) and adherence to clinical protocols
- Drive integration between OTP services and primary care, including warm handoffs, co-located screening (HIV, HCV, TB, STI), chronic disease management, and referral loop closure
- Lead site quality improvement activities, incident review, root cause analysis, and performance improvement plans
- Monitor and report clinical quality and outcome measures including treatment retention, illicit use reduction, and patient experience to OTS leadership
- Recruit, hire, onboard, supervise, cross-train, and develop clinic staff including Nursing Director, nurses, Clinical Supervisor, counselors, medical assistants, front office, patient navigators, concierges and patient transporters
- Conduct performance management, coaching, and annual reviews; build bench strength and reduce single-point-of-failure roles
- Foster a culture of accountability, trauma-informed care, harm reduction, and cultural humility
- Manage site budget, labor costs, supply spend, and productivity targets
- Partner with revenue cycle to ensure accurate documentation, coding, charge capture, and payer compliance across Medicaid, IHS/tribal, commercial, and grant-funded payment sources.
- Support grant reporting, data submissions, and funder deliverables
- Partner with OneTogether Solutions (OTS) Regional Directors and Account Managers to develop, track, and report outcomes of services.
- Serve as the operational point of contact for tribal leadership, tribal council designees, and community partners
- Represent the clinic with referral sources including tribal health programs, courts, corrections, child welfare, hospitals, and community-based organizations
- Support sovereignty transition planning and knowledge transfer as applicable to the partner relationship
- Bachelor's degree in healthcare administration, nursing, social work, public health, or related field
- Minimum 5 years of progressive healthcare operations experience, including at least 2 years in a leadership role
- Demonstrated experience leading or supporting a successful accreditation survey (CARF, Joint Commission, AAAHC or equivalent)
- Experience with integrated primary care, FQHC, community health center, or wraparound service delivery models
- Proven track record of managing multidisciplinary clinical teams
- Ability to pass background check, drug screen, and any tribal, state, or federal credentialing requirements
- History of working within a mental health or substance use disorder system, including the provision of medications for opioid use disorder highly desired.
- Willing to follow and train staff in Harmony Health Center's personnel policies.
- Able to work the hours and days required by position, including daily 5:30 am start times, from time to time.
- A successful track record of working with, and sensitivity to, multi-cultural populations.
- Master's degree (MHA, MPH, MBA, MSW, MSN)
- Active clinical license (RN, LCSW, LPC, LMFT, LAC) or CADC/LADC credential
- Experience operating a licensed Opioid Treatment Program, including working knowledge of 42 CFR Part 8 and 42 CFR Part 2
- Registered with the California Board of Behavioral Sciences or certified (or registered) by a California-recognized drug and alcohol certifying body.
- Experience working with tribal health programs, IHS, or Public Law 93-638 self-determination contracts
- Experience with EHR optimization in a behavioral health or OTP setting (e.g., Methasoft, Smart Management, Athena, Epic)
- Experience with harm reduction programming, syringe services, or naloxone distribution
- Bilingual or fluency in the language of the community served
