SUBJECT: JOB DESCRIPTION – BEHAVIOR SERVICES COORDINATOR

REPORTS TO: Case Manager Director

PRIMARY FUNCTION: Develop, coordinate, and monitor each person’s behavior program and compile reports and data sheets related to behavior programs for all programs.

RESPONSIBILITIES

1. To coordinate with Behavior Analyst, IDT, COS and Case Managers to monitor and compile reports and data sheets related to behavior programs.

2. Complete behavior assessments as indicated and draft behavior plans.

3. To write staff instructions for behavior programs and outcomes for ICF.

4. To coordinate the development of data sheets that are needed to implement the behavior programs as indicated.

5. To train new and existing staff on behavior plans and programs for all people supported.

6. To review behavior data in all programs, as indicated.

7. To monitor the effectiveness of Behavior Programs through on-site observations, monthly reviews and through communication with the IDT and COS. Coordinate any changes that are indicated.

8. To write monthly reviews for behavior services for ICF and coordinate any changes that are indicated.

9. To compile data and attend quarterly psych clinics.

10. Take On-Call responsibilities as part of management team on a rotation basis.

11. To perform other functions as requested.

QUALIFICATIONS

Credentials

High School Diploma or GED.

At least one year of experience with people with Developmental Disabilities.

Ability to be insured as a vehicle driver by agency's insurance program and a valid driver's license.

Good knowledge of basic English sufficient to read instructions, keep records and write reports.

Successful completion of background and reference check and drug screening.

Physical Requirements

Ability to intervene physically on an occasional basis with aggressive and violent peoples including, but not limited to, the ability to engage in quick and exertive holding, stooping, bending, twisting, and turning.

Ability to physically assist people in daily activities, including, but not limited to, assistance in transfers, eating, bathing, dressing, and toileting.

Ability to perform repetitive bending and exertion movements daily including, but not limited to, those found in bed making, lifting laundry, and mopping.

Mobility sufficient to climb stairs daily and easily, enter and exit vans daily, and occasionally engage in running.

Ability to engage in physical exercise several times a week with people including, but not limited to walking, shooting basketball and pool exercise.

Visual and auditory ability to meet peoples' needs and assure their safety.

Emotional Requirements

Ability to handle appropriately the stress which is created by job situations including, but not limited to, noise, conflicting demands for time and attention, and physically and/or verbally aggressive/combative people.
