Hybrid Lead Care Manager ECM and Community Health
Heritage Health Network
Heritage Health Network (HHN) is seeking a Lead Care Manager to support members through our Enhanced Care Management (ECM) program in the High Desert region, with a hybrid schedule (2 days remote, 3 days in the field) for Victorville-based work.
As the primary contact for members, you will develop personalized care plans with providers, coordinate care with healthcare teams and community resources, educate and advocate, address gaps in care, and maintain accurate documentation.
