Senior RN Care Manager Chronic Care & IDT Lead (Hybrid)

The RN Care Manager provides clinical leadership for care management in accordance with NCQA standards, CMS guidelines, health plan contracts, and company policy. The RN conducts comprehensive clinical assessments, medication reconciliation, evaluates LVN data, and designs SMART care plans for individualized care. The role leads IDT reviews, collaborates with multidisciplinary providers, and ensures safe transitions of care while managing complex chronic conditions and reducing hospitalizations. #J-18808-Ljbffr
