Nurse Practitioner / Physician Assistant

Location: Albany, NY

Schedule: Monday–Friday, flexible hours

Employment Type: Full-Time

Job Summary

Seeking a Physician, Nurse Practitioner, or Physician Assistant to provide wound care services in skilled nursing and long-term care facilities. No prior wound care experience is required; training is provided.

Responsibilities

Conduct patient assessments and weekly facility rounds.
Diagnose, treat, and monitor wounds.
Perform debridement when clinically indicated.
Collaborate with facility staff and coordinate referrals.
Maintain accurate patient records using EMR.

Requirements

MD, DO, NP/CRNP, or PA qualification.
Applicable board certification or eligibility.
Active, unrestricted New York State license.
DEA registration, as applicable.
Valid driver's license and strong documentation skills.
