Overview of Job The Registered Nurse (RN) supervises and coordinates the efforts of nursing staff and provides total nursing care for residents. This direct resident care role requires compliance with California Code of Regulation Title 22 and the State Health and Safety Code. Duties and Responsibilities Manage area of responsibility to achieve and maintain the highest quality of care. Monitor employees to ensure overtime is not utilized without prior approval. Contact physicians as necessary to ensure timely and appropriate interventions. Supervise nursing unit, including scheduling and directing staff to the extent permitted by the state practice act. Monitor employee meals and 10‑minute breaks to assure compliance. Monitor food carts to ensure meals are served in a timely manner. Coordinate effective use of personnel, supplies, equipment, and facilities. Monitor nursing care for residents throughout the shift. Recommend purchase of materials or equipment to improve nursing services. Monitor noise level on the unit to keep it minimal. Participate in the nurse on‑call system. Review staffing patterns and census of nursing units; reassign personnel when necessary to meet resident needs and budgetary controls. Obtain medications, supplies, and medical records needed for safe, efficient, and therapeutic care. Ensure monthly order recapitulation is completed accurately for the assigned unit/area. Monitor skills of new staff; guide and observe staff needing additional assistance with procedures. Develop, implement, and evaluate programs that promote recruitment, retention, development, and continuing education of nursing staff. Complete capitulation summary for discharged residents within five days of discharge. Supervisory Responsibilities Supervise, direct, and evaluate assigned staff members, LVNs, and CNAs. Assign nursing care to team members based on resident needs and individual capabilities and qualifications. Evaluate data and compare courses of action when assigning team members. Conduct or participate in performance evaluations of CNAs under supervision. Direct nursing staff to the extent permitted by the Nurse Practice Act. Meet with unit personnel throughout shift to identify and correct problem areas; provide shift reports at the beginning and end of the shift. Coordinate nursing care when residents are scheduled for therapy or procedures by other departments. Interview applicants and recommend hires. Ensure nursing personnel follow established procedures and company policies. Correct, coach, and counsel underperforming personnel. Participate in counseling, disciplinary action, and termination procedures to the extent permitted by the Nursing Practice Act. Investigate accidents and unusual occurrences; report findings to the Director of Nursing Services (DNS). Investigate complaints and grievances; report findings to DNS. Keep DNS informed of resident, staff, and related matters status. Promote interpersonal relationships and develop staff morale. Assist with or institute emergency measures for sudden adverse developments in residents. Take overall authority and responsibility for directing services on the assigned unit/shift. Account for performance of tasks by LVNs and CNAs under RN supervision. Documentation Function Maintain accurate and up‑to‑date medical records, including nursing assessments, care plans, and progress notes. Document interventions, observations, and resident responses to treatment. Conduct weekly chart audit for all patients assigned to the unit (all assessments, MARs, flow sheets, and treatment sheets). Ensure appropriate and timely documentation of resident care activities. Education Support Provide education and support to residents and their families regarding health conditions, medications, and self‑care. Assist in implementing therapeutic activities and programs. Recommend plans for staff training and development using quality assurance indicators, incident report investigations, and direct observation. Staff Development Assist the Staff Development Director in developing, planning, conducting, and scheduling in‑service training classes that provide instructions on ‘how to do the job’ and ensure a well‑educated nursing service department. Ensure training programs meet continuing education requirements for licensed personnel (RNs, LVNs, CNAs, etc.) to maintain active licenses. Assist in the orientation of newly assigned personnel and supervise/instruct other nursing personnel as needed. Safety and Sanitation Develop safety standards for the nursing service department. Adhere to all relevant healthcare regulations and facility policies. Participate in development, implementation, and maintenance of procedures for reporting hazardous conditions or equipment. Monitor personnel to ensure compliance with safety regulations in the use of equipment and supplies (e.g., back brace, mechanical lifts). Ensure a safe and clean environment for residents and staff. Resident’s Rights Maintain confidentiality of all resident care information. Ensure residents are treated fairly, with kindness, dignity, and respect; provide privacy and knock before entering a room. Ensure nursing personnel are knowledgeable of residents’ responsibilities and rights, including the right to refuse treatment. Working Conditions Work throughout the nursing service area (drug rooms, nurses’ stations, resident rooms, etc.). Move intermittently during working hours. Experience frequent interruptions. Interact with residents, personnel, visitors, government agencies, etc., under all conditions. Handle hostile and emotionally upset residents, family members, personnel, and visitors. Communicate with the medical staff, nursing personnel, and other department supervisors. Work beyond normal hours, weekends, and holidays when necessary. Be subject to call‑back during emergency conditions. Participate in community/civic health matters/projects as appropriate. Attend and participate in continuing educational programs. Be subject to injury from falls, burns, odors, dust, disinfectants, and tobacco smoke. Expose to infectious waste, diseases, including TB, AIDS, and Hepatitis B viruses. May be required to handle hazardous chemicals. Physical and Sensory Requirements Move intermittently throughout the work day. Coping with mental and emotional stress of the position. See and hear or use prosthetics to ensure senses function adequately. Work independently with flexibility, personal integrity, and effective collaboration with residents, personnel, and support agencies. Meet general health requirements per facility policies, including a medical and physical exam. Relate to ill, disabled, elderly, emotionally upset, and occasional hostile persons within the facility. Push, pull, move, and/or lift a minimum of 50 pounds to a minimum height of 3 feet and carry that weight a minimum distance of 10 feet. Assist in evacuation of residents during emergency situations when needed. Qualifications / Requirements Education: Associate’s or Bachelor’s degree in Nursing. License: Registered Nurse (RN) license in the state of employment. Must be able to read, write, speak, and understand English. Must possess the ability to make independent decisions when circumstances warrant. Must deal tactfully with personnel, residents, family members, visitors, government agencies, and the general public. Must work harmoniously with other personnel. Must have patience, tact, a cheerful disposition, enthusiasm, and be able to handle difficult residents. Willing to seek new methods and principles and incorporate them into nursing practices. Able to communicate information concerning a resident’s condition. Must not pose a direct threat to the health and safety of others. Knowledge of training techniques for clinical staff. Demonstrate competence in nursing skills within the Nurse Practice Act expectations. Ability to direct and motivate the workforce, including building, leading, and managing the team.
