Licensed Practical Nurse (LPN) - Long Term Care

LPN – Nursing

Schedule: Full Time, Day/Evening rotation (5x8s), every other weekend and holiday required.

Assignment Details: Position involves providing care to residents in a healthcare facility.

Requirements:

• Current NY LPN Licensure
• BLS certification through AHA or RQI
• Minimum of 1 year of experience as an LPN

Responsibilities:

• Collect and assess data relevant to residents' physical and psychosocial conditions, communicating findings to the RN and interdisciplinary team.
• Contribute to the development of culturally relevant care plans involving residents and families.
• Reevaluate care plans and recommend revisions as needed, ensuring all aspects of care are addressed.
• Maintain current clinical knowledge and participate in the mentoring and orientation of new staff.
• Provide leadership under the direction of the RN while ensuring quality care delivery.
