Work Shifts 9:00 A.M - 5:00 P.M (Monday - Friday) Duties & Responsibilities PURPOSE OF POSITION: Under the supervision of the Associate Director, the Assistant Director of Risk Management for Behavioral health aids in planning, organizing and implementing the goals and mission of the BH Risk Management Department. The goals are to identify, evaluate and reduce or eliminate the risks of harm to all patients and staff, to facilitate the provision of high-quality patient care through the reduction of risks of harm and to reduce the risks of financial losses to the hospital and system-wide. SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:- Review incident reports and provides analysis/summary of findings/ Resolutions; Reviews all sources of pre-litigation referrals for cases that are potentially reportable to the New York State Justice center, NYS Office of Mental Health, Patient Occurrence and Tracking System (NYPORTS) and other regulatory agencies.
- Follow established guidelines for the reporting and closing of adverse events to the Office of Mental Health (OMH) NIMRS and Department of Health (DOH) New York Patient Occurrence Reporting and Tracking System (NYPORTS), within the designated time frames.
- Investigate events that have led to patient harm or have the potential to lead to patient harm; Facilitates /Conduct root cause analysis investigation; Conducts chart and medical record reviews and prepares summaries of findings; refers patient quality of care issues to the appropriate personnel or department for additional review and analysis.
- Provides on-going monitoring of identified potential risks that exist in the areas of hospital operations or in the rendering of medical and nursing care.
- Assists in the preparation of quarterly Board of Directors reports regarding occurrences and investigative findings, including actions taken to prevent/reduce reoccurrences and improve performance and patient safety.
- Collaborate with Central Office Legal Affairs as assigned; May prepare correspondences or other documentation, as directed, for Counsel in the medical litigation division, outside Counsel retained by the Corporation
- Reviews highly complex risk cases and documents the appropriate actions to take in order to rectify the issue at hand to avoid litigation.
- Attends and participates in committee meetings, SIRC (Special Incident Review Committee), RCAs and board report prep meetings and performs special projects as required; Coordinate and schedule meetings/minutes for assigned meetings
- Follow up on risk reduction strategies (RRS), both implementation and effectiveness; Identify trends to be addressed with individual units/departments.
- Accept and process subpoenas from process servers, Supreme Court, ADA, Criminal Court; Process all Notice of Claims maintain files electronically and hard copies. Maintain Notice of Claims spreadsheet data.
- Assist in developing and conducting training on Incident & Risk Management
- Assist in reviewing, formulating and implementing incident management or risk management related policies
- Perform other activities as assigned by BH Associate Director.
1. A valid license as a New York State Registered Professional Nurse and two (2) years of satisfactory nursing experience in a hospital environment; or
2. A valid New York State registration as a Physician's Assistant and one (1) year of
satisfactory experience as a Physician's Assistant in a hospital environment; or
3. A baccalaureate degree from an accredited college or university and two (2) years of
responsible full-time health care risk management experience in a legal or medical
office/division; or
