A well-established, mission-driven hospice organization serving the Muskogee area is seeking a compassionate, dedicated Licensed Social Worker to join our interdisciplinary team. What You'll Do As a Hospice Social Worker, you'll play a vital role in helping patients and families navigate the emotional, social, and practical challenges that accompany serious illness by: Providing compassionate counseling and emotional support to patients and their loved ones. Completing psychosocial assessments and developing individualized care plans. Connecting patients and families with community resources, financial assistance, advance care planning, and supportive services. Advocating for patients while serving as a trusted guide throughout the hospice journey. Collaborating closely with nurses, physicians, chaplains, aides, and other interdisciplinary team members to deliver exceptional, patient-centered care. Supporting families through anticipatory grief, bereavement, and difficult life transitions. What You'll Bring Required Qualifications Master of Social Work (MSW) from a CSWE-accredited program. Current Oklahoma Social Work license (LMSW or LCSW preferred). Minimum of one (1) year of healthcare experience (hospice, home health, palliative care, or related setting preferred). Excellent communication, organization, and interpersonal skills. Current CPR certification (or willingness to obtain). Valid driver's license, reliable transportation, and current automobile insurance. Why Join Us?
