Platinum Strength PT is a growing physical therapy company bringing convenient, high-quality care directly to senior living communities across Chicago.

We are looking for an entrepreneurial, motivated Physical Therapist who wants more than a traditional clinic role. This is an opportunity to join at the ground floor of a new care model, work closely with the founder, help shape clinical operations, and grow alongside the company as we expand into additional communities.

The role will begin on a flexible, part-time basis, with the potential to grow into a substantial full-time position as patient volume increases. For the right person, there may also be opportunities for leadership responsibilities, performance-based compensation, and meaningful long-term upside as the organization grows.

The ideal candidate is clinically strong, comfortable working independently, enjoys working with older adults, and is excited by the opportunity to help build something from an early stage.

What You’ll Do

• Provide one-on-one outpatient physical therapy services to residents within senior apartment communities.
• Perform evaluations and develop individualized plans of care focused on mobility, strength, balance, pain, fall prevention, and functional independence.
• Deliver evidence-based treatment and monitor patient progress.
• Complete accurate, timely clinical documentation.
• Educate patients on exercises, mobility strategies, and ways to maintain independence.
• Participate in community wellness, strength, balance, and fall-prevention programming.
• Communicate effectively with patients, families, physicians, and other members of the care team when appropriate.
• Help establish clinical workflows and best practices as Platinum Strength continues to grow.
• Potentially take on increased clinical and leadership responsibility as additional locations and therapists are added.

Why This Role Is Different

This is not a traditional high-volume outpatient clinic position.

Platinum Strength is building a community-based physical therapy model designed to make care easier and more accessible for older adults. Instead of patients traveling to a clinic, we bring care directly to the communities where they live.

As an early clinical hire, you will have the opportunity to help shape how the program operates rather than simply stepping into an established system. As the company expands, there may be opportunities to:

Qualifications

• Current Illinois Physical Therapist license or eligibility for licensure
• Doctor of Physical Therapy degree or equivalent
• Strong clinical judgment and patient communication skills
• Ability to work independently
• Comfortable with clinical documentation and plan-of-care development
• Interest in working with older adults
• Outpatient, geriatric, home-based, acute care, or rehabilitation experience preferred
• Medicare Part B experience is a plus, but not required
• Familiarity with physical therapy billing and CPT coding is helpful
• Reliable transportation for travel between Chicago-area communities

Schedule

Flexible, part-time initially, with the opportunity for increasing hours as patient volume grows.

Compensation

Competitive hourly compensation based on experience, with the potential for performance-based bonuses and additional long-term opportunities as the company expands.

Pay: $55.00 - $70.00 per hour
