Minimum Requirements

Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Ohio Physical Therapist license required to start the assignment.
BLS certification from the American Heart Association or American Red Cross may be required.
Minimum 12 months of home health experience within the past 3 years (client requirement).
Experience with HCHB and OASIS documentation systems (client requirement).
Ability to meet productivity expectations of 30 points per week and maintain a 40 hours per week schedule as required by the client.
Reliable transportation and ability to perform home visits in the local service area.

Shift Name: Days.
Shift Details: Monday – Friday, 8:00 AM – 5:00 PM; 5x8 schedule.
On-call: Not specified.
Length: 13 weeks.
Must meet client productivity expectations of 30 points per week and availability to work up to 40 hours per week as guaranteed by the client.
