Licensed Practical Nurse (LPN) - Bradford
Job Summary:

Responsibilities:
Provide direct patient care to our residents, including assessments, medications, treatments, and procedures
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Maintain accurate and detailed records of patient treatments, medications, and progress
Report patient concerns and changes to the registered nurse or supervisor
Participate in quality improvement initiatives to enhance patient satisfaction and outcomes
Collaborate with other departments to ensure seamless care transitions and coordination
