Beth Israel Deaconess Medical Center is expanding its Advanced Heart Failure and Cardiac Transplant Pharmacy Services and is seeking a Clinical Pharmacy Specialist to support both inpatient and ambulatory care across these service lines.
The Cardiac Transplant Clinical Pharmacist will rotate through inpatient heart transplant and advanced heart failure services as well as ambulatory care, serving as a subject matter expert in cardiac transplant and advanced heart failure pharmacotherapy for interdisciplinary teams across the continuum of care. This role involves applying advanced training to lead immunosuppressive management and opportunistic infection discussions during interdisciplinary rounds, provide comprehensive transplant-related education to patients and healthcare professionals, and contribute to the development and maintenance of transplant protocols, guidelines, and clinical pathways in both inpatient and outpatient settings. The Clinical Pharmacy Specialist will also participate in quality improvement initiatives, research activities, and precepting responsibilities, including training and mentorship of PGY-2 residents and other pharmacy learners. The Cardiac Transplant Clinical Pharmacist is recognized as a key clinical resource within the Pharmacy Department and collaborates closely with the Cardiac Transplant, Advanced Heart Failure, and Cardiology teams to optimize patient outcomes across the continuum of care.
Essential Responsibilities:
- Practices pharmacy in accordance with laws, rules and regulation of the Massachusetts Board of Registration in Pharmacy, Department of Public Health, U.S. Food and Drug Administration, and in accordance with established departmental policies and procedures. Maintains credentials in accordance with the Massachusetts Board of Registration in Pharmacy.
- Functions as a therapeutic expert and performs pharmacy consultations per policy guidelines. Works collaboratively with providers to provide guidance on appropriate use of medications, clinical pharmacology, pharmacokinetics, disease state management, and nutrition. Recommends evidence-based medication therapy regimens and monitoring plans.
- Works collaboratively with the clinical pharmacy coordinators to develop and implement evidence-based medication guidelines/standard operating procedures/policies. Attends and actively participates in hospital committees pertinent to specific practice area.
- Enters/verifies medication orders in the computer processing system accurately and efficiently. Ensures appropriateness of all medication doses in reference to patient's diagnosis, weight/body surface area, age, allergies, and drug interactions. Clarifies incomplete and unclear orders. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed per formulary guidelines.
- Precepts PGY1 and PGY2 pharmacy residents, pharmacy students, and contributes to staff development in area of specialty practice. Works collaboratively with the clinical pharmacy coordinator to develop the pharmacy skills of pharmacy staff members by teaching and exemplifying those skills to the team members.
- Provides educational information/seminars to physicians and other Licensed Independent Practitioners (staff), nursing staff, and other allied health care professionals as needed or assigned. Facilitates collection and analysis of medication-use data per predefined criteria and formulates appropriate recommendations for implementation.
- Provides patient consultations, assessments, and educations as needed to meet patient care needs. Documents patient care activities appropriately in a manner specified by the pharmacy department. Oversees and directs pharmacy support personnel including the daily activities of technicians.
- Serves in a professional capacity on unit-based decentralized teams. Acts as a liaison to the nursing units, in assigned practice area, in order to communicate pertinent information and to facilitate communication. Accurately re-establishes priorities based on workload and patient needs and ensures that medication doses are delivered within established departmental guidelines.
Required Qualifications:
- Bachelor's degree in Pharmacy required. Doctoral degree in Pharmacy (PharmD) preferred.
- License Massachusetts Pharmacist required.
- 1-3 years related work experience required.
- 1 year Pharmacy residency or 3 years of Clinical Pharmacy experience in hospital setting.
- Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.
Preferred Qualifications:
- Pharmacy Residency PGY2 specialty experience or equivalent experience or 5 years of clinical pharmacy experience in a hospital setting.
- Board certification in a Pharmacotherapy specialty.
- Project management and program development experience a plus. Pharmacy residency preceptor experience preferred. Coaching and mentoring of pharmacy staff experience preferred. Demonstrates advanced pharmacy knowledge related to area of practice.

- Knowledge: Ability to demonstrate in-depth knowledge of concepts, practices and policies with the ability to use them in complex varied situations.
- Team Work: Ability to act as a team leader for small projects or work groups, creating a collaborative and respectful team environment and improving workflows. Results may impact the operations of one or more departments.
- Customer Service: Ability to provide a high level of customer service and staff training to meet customer service standards and expectations for the assigned unit(s). Resolves service issues in the assigned unit(s) in a timely and respectful manner.
Age based Competencies: Employees in this job must be competent to provide patient care to the following age groups: Neonatal: Birth to 6 months, Youth: 6 months to 16 years, Young adult: 16-30 years, Middle Age: 30 - 60 years, Elderly: 60 -. Physical Nature of the Job: Light work: Exerting up to 20 pounds of force frequently to move objects. Some elements of the job are sedentary, but the employee will be required to stand for periods of time or move through out the hospital campus
Pay Range: $127,987.00 USD - $162,318.00 USD
The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
