Join Priority Care Rehab Staffing 's SNF clinical team helping post-acute and long-term care residents regain independence in daily living. You'll collaborate with an interdisciplinary team focused on meaningful functional outcomes.
Responsibilities:
- Evaluate residents and develop individualized Plans of Care targeting ADL and functional goals
- Provide skilled OT interventions including ADL retraining, cognitive rehab, fine motor, and UE strengthening
- Maintain timely documentation in compliance with Medicare, Medicaid, and facility requirements
- Collaborate with IDT (physicians, nursing, PT, SLP) and participate in care conferences
- Supervise COTAs per NYS OT practice guidelines and support safe discharge planning
Requirements:
- Active New York Occupational Therapist license
- Current CPR/BLS certification
- Prior SNF experience preferred
- Excellent communication, documentation, and team collaboration skills
