Join Our Compassionate Hospice Team!

Job Description

Department Overview: Join our dedicated hospice team that supports patients and their families during the final stages of life.
Service Area: Evergreen Hospice provides care in Linn, Benton, Marion, Polk, Lincoln, and Tillamook Counties.
Care Settings: Deliver services in various settings, including patient homes, skilled nursing facilities, assisted living facilities, and adult foster homes.
Comprehensive Services:
- Intermittent skilled nursing visits
- 24-hour on-call nursing services
- Pain and symptom management
- Medical social services
- Counseling and spiritual support
- Hospice aides and trained volunteers
- Bereavement support
- Medications and equipment provided as needed
Candidate Preference: Preference will be given to candidates with previous hospice experience.
Qualifications:
- Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure required.
- Healthcare Provider Level BLS certification required.
- Valid driver's license and auto liability/property damage insurance are a must.
- Oregon LCSW (Licensed Clinical Social Worker) is preferred.
- Experience as a social worker in a hospital or medical setting is preferred.
Work Schedule: Days (0800-1630), no weekends!
