As a Social Worker in this role, you'll provide essential support and guidance to patients and their families, ensuring they receive the care and compassion they deserve.

In this rewarding position, you'll be responsible for:

Providing emotional and social support to patients and their families
Collaborating with healthcare professionals to develop comprehensive care plans
Offering counseling and spiritual support as needed
Coordinating with hospice aides and volunteers to ensure holistic care
Managing pain and symptom relief strategies
Utilizing your personal vehicle for patient visits

Requirements for this role include:

Oregon LMSW or CSWA licensure required
Healthcare Provider Level BLS certification
Valid driver's license and auto insurance
Experience as a social worker in a hospital or medical setting preferred
Need 1 year experience in setting
