(Required) Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent degree. (Required) Proof of current CPR certification. (Required) Current driver’s license, reliable transportation, and vehicle insurance. (Required) 1–2 years of experience as a Physical Therapist in an acute care or rehabilitation setting. (Required) Previous home health experience. (Preferred) Demonstrated knowledge of durable medical equipment and ability to order and adapt rehabilitative equipment for home use. (Client-specific requirement) Ability to perform clinical assessments, prepare and revise Plans of Care (POC), document progress notes and phone orders, and collaborate with interdisciplinary teams. (Client-specific requirement) Ability to perform patient/family education, instruct on transfers and assistive devices, and provide treatments for neuromuscular, neurological, and orthopedic diagnoses. (Client-specific requirement) Ability to lift, bend, push, pull, and perform prolonged standing and walking as job duties require.
