Field-Based Case Manager - Family & Mental Health Support
Wellnest
Wellnest is seeking an experienced Case Manager to support children, teens and families in promoting emotional well-being. You will conduct group sessions, assess needs, connect clients to health insurance, housing, and low-cost child care, and coordinate care with therapists and other agencies. You will document progress in the agency's EHR, maintain a strong focus on family empowerment, and work in field-based settings including homes and schools. #J-18808-Ljbffr Wellnest
