Competitive compensation with achievable incentive bonus program To be a success a Registered Nurse (RN) Home Health Case Manager, Level 2: One Year of Home Health Case Management experience is perferred Minimum of one year of Nursing Experience required; two+ years preferred. Experience supervising, mentoring Nurses is highly preferred Current and unrestricted CA RN license Current BLS/CPR Certification Valid CA Driver's License High level of clinical knowledge in home health care is key Experience with high acuity patients is a plus Experience working with an interdisciplinary team is required Case Management fundamentals is required Self-Directed Empathetic Critical thinker Outstanding interpersonal skills Leadership skills Organized By providing your contact information, you agree to receive communications from 21st Century Home Health Services. Target Wage range Registered Nurse, Level 1: 140,000.00- 160,000.00 plus 20,000.00 Annual Bonus, paid quarterly.
