PRN Home Infusion Nurse

Job Overview An Infusion Nurse I is a licensed registered nurse who provides basic direct patient care to patients in the home or at alternate infusion sites. The role ensures patient safety and compliance with a physician’s plan of care while working under close direct supervision of the designated supervisor. Key Responsibilities Initiate, develop, and implement nursing plans of care, and evaluate patient progress toward goals. Provide direct patient care, perform treatments, administer medications, and educate patients and families/caregivers. Modify treatment plans in response to changing patient status based on physician orders. Assess patient needs and physical status at each visit through data access and patient interview. Reevaluate patient needs through physical reassessment, response to therapy, and supplemental physician orders. Obtain and clarify physician orders, inform physicians promptly of significant changes, and provide written summaries. Deliver effective, patient‑centered teaching to patients and families for independent therapy management. Communicate with Naven Health’s clinical team, medical providers, patients, and families to facilitate continuity of care. Prepare and submit real‑time clinical documentation in compliance with billing timelines. Coordinate discharge planning, prepare discharge summaries, and report patient needs and progress during care transitions. Demonstrate compliance with agency policies, professional standards, and regulatory guidelines. Use computers and automation technologies to validate orders and manage the visit schedule. Respond promptly to patient requests and initiate emergency procedures when necessary. Maintain ethical and professional conduct, safeguarding patient confidentiality per HIPAA regulations. Attend and complete required training, in‑services, and continuing education to maintain competency and licensure. Provide oversight of paraprofessionals in the home setting and act as supervisor in the absence of the supervisor or nurse manager, as permitted by regulations. Participate in multidisciplinary team conferences, precepting, training, and mentoring of other nurses. Participate in nursing on‑call responsibilities as applicable. Perform other duties directed by the supervisor. Supervisory Responsibilities This position does not involve hiring, promotion recommendations, scheduling, performance reviews, or disciplinary actions. Basic Education and Experience Active and unrestricted RN license in the state of practice. Minimum of 0–2 years of infusion nursing experience. Current CPR certification (in‑person class required per company policy). Advanced certification and training as applicable. Valid driver’s license and access to a vehicle with proof of liability insurance. Basic computer skills and ability to use nursing technology tools for documentation and communication. Physical Demands Lifting up to 50 lbs with occasional up to 75 lbs. Exerting up to 20 lbs of force occasionally, or up to 10 lbs frequently. Walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting, and carrying. Fine motor skills and visual acuity, including color vision and depth perception. Risk of exposure to blood, body fluids, infectious materials, hazardous substances, needles, sharps, fumes, chemicals, humidity, cold, heat, adverse weather, animals, secondhand smoke/vape, and unpredictable home environments. Safety requirements include closed‑toe shoes, face mask, goggles, gown and/or gloves, and functional communication devices for routine and emergency use. Travel Requirements Willingness to travel 100% of the time for business purposes. Preferred Qualifications and Interests Bachelor of Science in Nursing from an accredited school (preferred). One year of prior RN experience in a home health or acute care setting. Salary and Benefits Salary is determined by education, experience, and internal equity.
