RN Program Coordinator - Heart Failure/LVAD

Overview The HVI Heart Failure / LVAD Coordinator is a registered nurse who oversees current initiatives in cardiac care. Within the heart failure program this position collaborates with various departments/physicians/staff to ensure heart failure patients receive the highest quality of care possible and oversees certain heart failure related meetings. Assists with and coordinates data management and analysis of aggregate clinical outcomes as per GWTG and DNV performance measures for heart failure care. The Heart Failure / LVAD Coordinator collaborates with the medical director of the heart failure program and the heart failure program manager to ensure compliance for DNV requirements for current level of accreditation. The scope of the role includes coordinating information flow among medical, ancillary, and both inpatient and outpatient nursing teams. Acting as a liaison with the heart failure patient population and community care providers in order to match patient clinical needs to program/procedural offerings. In collaboration with Heart Failure Dyad medical leadership, leads the heart failure program quality and performance improvement initiatives to achieve successful patient outcomes. The Heart Failure / LVAD Coordinator educates the patient and family, staff and community as required, in conjunction with HVI clinical teams. Qualifications Certifications: Basic Life Support (BLS) within 30 days - American Heart Association (AHA); Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR); National Nursing Certification within 3 years - Varies, Education: Bachelor's Degree: Nursing, Work Experience: Responsibilities Participate in the development and maintenance of Heart Failure / LVAD statistical databases patient data. Support the Heart Failure / LVAD Program Committee quality and performance improvement initiatives, educates the patient and family, staff and community as required, in conjunction with HVI clinical teams. Work in a multi-disciplinary environment to promote quality in clinical practice for the Heart Failure / LVAD patient population. Coordinate information flow among medical, ancillary, and nursing. Program development as it relates to the Heart Failure / LVAD program, including management of relevant accreditation processes. Keeps heart failure leadership appraised of barriers to meeting regulatory standards including, but not limited to: DNV, ISO 9000, IDPH, CMS, LVAD Registry, and collaborative research. Leads Carle to maintain quality initiative surrounding heart failure. Provides specialty knowledge, input and oversight of the heart failure data collection/reporting and analysis of the heart failure program. Direct entry of heart failure clinic patients for analysis, data collection, and reporting. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
