Behavioral Health Case Manager

Talbert House, Inc. in Cincinnati, OH is seeking a Case Manager for Community Outpatient Behavioral Health to deliver comprehensive client care.
This full-time professional role covers assessment, planning, implementation, and ongoing evaluation of services to meet health and human service needs. You will coordinate with offsite supports, maintain caseloads, develop individualized service plans, document activities, and collaborate with a treatment team to promote client wellness and autonomy
